A few suggestions

Be constructive. I appreciate all feedback, and I'm more likely to act on it if it's kind.
Be specific. Concrete examples are more useful than general complaints. "Your meetings waste time" is harder to act on than "you spent most of Tuesday's call off-topic."
Make it actionable. Point to something I can actually do differently.
